How Do I Increase my FT?

My protocol is 75mg Test E twice a week for a total of 150mg. IM injection(glutes). No AI.

My labs after 6 weeks is quite crowded but i only want to discuss my FT.

I am confused as to why my FT is still low compared to my high TT.
My SHBG combined with my Albumin and TT should be pointing to a much higher FT yet in lab results my FT still shows up as low-mid value. Here they are below (trough levels):

Testosterone total 9.80 ng/mL 2.49 - 8.36
FT total 27.84 pg/mL 5.4 - 40
SHBG 18.5
Albumin 4.3
Estradiol (E2) 66.53 pg/mL 11.3 - 43.2

BTW 1 month ago with 60mgx2 protocol my SHBG was 31.5 , TT was around 750 , E2 was 29 and FT was 17.3

I don’t feel too bad yet i don’t feel like i am benefiting that much from TRT. A lot of room for improvement on my energy and especially how i wake up tired despite 8-9 hours of sleep.

How do i increase my FT ?
Should i up my dosage? Should i worry about my E2 if i do so? As far as what i have seen on this forum lately , above the range E2 is fine unless it’s extremely high.
Also twice a week is the best/most i can do for frequency of shots.

This has got to be a lab error because your FT doesn’t decrease when SHBG has decreased. It’s common for those with high estrogen levels to make the comment that TRT isn’t showing any positive results, in fact I have experienced it myself, that whenever estrogen is elevated, the benefits of TRT are seem to be gone.

If your thyroid is subpar, testosterone cannot be metabolised in the liver. Your have previous threads showing low T3/fT3 in addition to higher rT3. If your thyroid levels aren’t optimal, TRT will never show any benefits. If you don’t have any thyroid testing since September when we covered your thyroid issues, then we have a big hole to fill and a lot of unanswered questions.

The liver secretes estrogen, fT3 drives this process. You can’t increase your FT without also increasing estrogen, but you can minimize the conversion of FT–>E2 by injecting smaller doses more frequently.

I didn’t want to post my thyroid levels yet as i have to wait 10 more days to receive my latest RT3 result.

After our last discussion i’ve had another test for RT3 and it was 160pg/mL down from 201.2 pg/mL. That is why i decided to get another test and see if i have to either go for T3 only or NDT.
Also , interestingly my TSH is lower and my FT3 is higher now. TSH is 1.38 now and FT3 is 3.11 pg/mL (2.0 - 4.4). But my T4 and FT4 is still too low , no change there.

On this thread i only wanted to discuss the ratio of my FT/TT but you are saying sub-optimal thyroid can prevent my FT to increase right? So my low FT is somewhat related to my FT3?

As for my E2 , some people here saying they have absurd number of E2 without symptoms… I guess everyone is different.

Actually my SHBG decreased but my FT and TT both increased (FT from 17 to 27). Though the highest increase by far is my E2. From 29 to 66. Difference is by switching from 60mgx2 to 75mgx2.

There is the issue however that some people cannot optimize thyroid due to adrenal issues, insufficient cortisol.

Do you think TRT helps balance adrenals?

You started T3 medication due to high RT3 right? How is that going for you?

I decreased the dosage a bit because I got hyper symptoms. I think from my adrenals

And how are you feeling now? Also what is your current dosage?
I am about to begin T3 only too and i really wanted to go with NDT.

@kemosi I was taking 6mcg but I got a little hypo symptoms and decreased to 3mcg, but basically ot 3 I feel the same as before started to take it.
The explanation is I might have adrenal issues that cause me difficulties with T3 due to inadequately distributed cortisol throughout the day
