High SHGB, Very Low E2 and Very Low Free T

Hi all New guy here. 50-year old guy living in the United Kingdom. Have been on testosterone therapy for 3 years and never felt optimised. Libido has constantly been very very low and recently my shbg has gone up to 70 . . I’ve noticed that I am more irritable and moody than I’ve ever been the libido has been zero… I’m having sleep issues waking four hours after I’ve gone to sleep every night. I’ve been having faster heart rate in waking and thinking could it be cortisol that could push up the shbg. My protocol for the last 6 months has been 100mg split between two injections. Previously I was doing three injections a week and 6-months before that I was doing four injection a week . What I’ve noticed is that my shgb has been going up slowly even though the frequency of my injections have been decreasing. Now my free t is so low , it’s probably causing all of the symptoms… I’ve tried boron to bring my shgb down but hasn’t done anything. I am fit , I’m healthy I workout 5 days a week I eat clean I do not drink alcohol or take other medication. I am getting my bloods done this Thursday to check what my red cell counts are. Would you you think increasing my testosterone dosage to 150mg once a week would help bring down my shgb?

Thanks for the reply. I’m waiting for the results. All they said was its low. I get the result on Thursday… So just saying I didn’t show you the calc and that was my free t result… What would be the plan to free up more free t so I can have a libido and to eliminate the other symptoms

You basically have low testosterone at 100 mg per week, I think you’ll need more than 150 mg.

Thanks for the reply… I appreciate it… My tsh was OK so can’t see the high shbg because of thyroid… Just need my free t high

Cortisol can increase SHBG and TRT lowers cortisol.

I wasn’t 100% on whether SHBG increased or decreased after exogenous T was introduced; my own SHBG increased after starting TRT so I went with this in error.

I read your article and am aware SHBG and Estrogen are inversely related, but wasn’t particularly aware of this until reading. This is also why I didn’t comment as to the estrogen, just asked what the blood results were.

I’ll back off the TRT forum. I have too much schoolwork to do to get proficient in hormones AND Computer Tech.

Interesting thanks for that. So would you think adding in some hcg might help reduce the shgb this freeing up some free t. Then hopefully the brain fog and the mood swings might go and may be a libido might return ?

My SHBG has increased on TRT but has more to do with being a type 2 diabetic.

Boron also did nothing to reduce my shbg. It seems to be really hit and miss.

I work out heavy and hard. I read this can effect liver enzymes… Could the rise in liver effect shbg ? Is there any good supps I can take to help the liver when working out ?

Thank you for that reply I really appreciate it. What you say ties in with what I’ve been looking at over my last two years test results. Over the last 2 years when my dosage was higher my shgb was lower… . So on that basis what what difference does frequency of injection do ?

Hi, this looks like an online calculator rather than an actual blood test?
Who do you use for your tests?
Surprised your getting low T on that dose, should enough for mid to high normal results in the absence of other complicating factors?

What are your estradiol and Prolactin levels like?

Brain fog and mood are relatively easy to fix with a good TRT regimen, Libido is much trickier, best not to get too fixated on that i think

I’m getting those on Thursday … I’ve reduced my injections to once a week but upped it to 125mg and it was 100mg… Red blood cells also getting checked on Thursday

Yeah as long as I have the other results free t can easily be worked out .

Been there too, with hindsight, nothing to do with Test levels and everything to do with general life/work/family related stress

Thanks for the reply… I’ve been the same since starting trt… The symptoms get worse when the the free test is low… I have no stress. I have a great life and I’m happy with what I have . Trust me this is all related to test. I know my body and how it reacts . I had stopped try for 7 months back in 2019 and I went thru hell… Hell with the worse symptoms ever … that’s had nothing to do with stress and life. It was because they stopped my test

The NHS doesn’t test free t… So I’ll have to pay private . Surley the free t calculation wouldn’t be that far off the mark ?
