Is it possible to have primary hypogonadism despite totalt testosterone being high, if SHBG (and LH) is elevated?
No. Primary indicates inability to produce testosterone via dysfunction in the testes. You would/could have high fsh/lh but little testosterone.
You can have neither secondary or primary hypogonadism, have high SHBG, high LH, high Total T and low Free T.
If this is the case, don’t waste time trying to fix it naturally, it’s futile unless it’s as simple as a medication causing the high SHBG. The high SHBG could be caused by starvation (cutting), which can increase insulin sensitivity to the point which causes a substantial increase in SHBG, which is what happens when a diabetic improves fasting glucose levels.
In these cases, your body is producing a lot of testosterone, but the majority of it is being deactivated and therefore your body can’t use it. Some find this condition irreversible and require TRT for life.

This question is impossible to answer with absolute certainty, there is no consensus at what point a person with experience low-T symptoms as there are too many variables, that being genes, CAG repeat lengths (long/short), current health problems and environmental factors.

If so, the answer is yes. I see this all the time. Had a new guy yesterday, total testosterone 1008 (250-1100) and free testosterone 52 (35-155). Miserable, going on three plus years. PCP finally agreed to test him (tested total testosterone only and then after putting him off for a couple years) and recommended psychotherapy. He’s going to be really pissed when he starts to feel good.
