You get these guys who have mild FH, their numbers are only slightly elevated.
Doctor might think they are fine, but particulate counts are all sorts of fucked up.
Bam… heart attack at 45-50-60.
Familial hypercholesterolaemia makes up roughly 20% of all heart attacks in men under the age of 45. It’s a woefully under recognised and under treated condition.

Variations in severity. Though i’d agree OP doesn’t have it.
My lipid profile is about the bare minimum for someone to consider the dx. Last reading was 160 ldl, 39 hdl and a total of around 225. That’s the worst it has ever been. My diet is pretty good.
I’ve had one doctor say “that doesn’t sound too bad” as this one was opposed to me being on statins after I was put on (I pushed for them… hard… due to my familial history particuarly on one side). But he was looking at my total… not my HDL, or ratio to LDL, trigs etc.
Both of my parents are on statins, and both have elevated lipids. One had an LDL of around 200 if I recall correctly. My grandfather apparently died of a stroke in his 40s… heavy smoker (40+/day apparently), ate badly… but exercised a lot
With FH that’s enough to do you in… without FH that might even be enough for some people (40/day for almost 30 years).
My aunt apparently has a total of around 350, has smoked her entire life… and still smokes.
She was put on statins in her 50s I think. Somehow she’s still alive in her 70s… how? I have no idea… she certainly has FH as it def runs in both sides of my family, so i’d be interested to know what has stopped her from having a heart attack/stroke decades ago.
An 18 year old who exercises 6 times a week, eats clean and in a deficit, weighs 165 lbs at 6 foot and has a total cholesterol of 240 does not have some form of HeFH?
My grandfather had his first heart attack at 40.
I am now 27 and still have cholesterol between 210-250 every reading weighing 190 lbs at 6 foot. It’s clear I don’t have a severe form, but don’t you think it’s very likely?
I don’t see another explanation for these labs.
I am well aware and I think I’ve even mentioned it in my original post.

Had no idea this was the particular scenario we were referring to.
I thought we were referring to like…
A 30 year old who lifts five times a week but otherwise works full time and adheres to eating a normal western diet.
In which case i’d say a 240 total is borderline
Under those circumstances for 19y/o me my total was around 203 or something (but low HDL, LDL was 135 on vegan diet, in deficit doing tons of cardio… literally hours a day)
I weighed 155 at that time. It was when i couldn’t lift weights at all due to a particularly damaging shoulder dislocation.
Just take statins…
