I got my results done privately and those results prompted me to go to the GP. He wanted to test again to have it on their system. Note I’m 20 years old with anxiety, depression, low energy for anything etc. (textbook symptoms?)
These were the notable results that were ‘abnormal’
Serum Testosterone 5.1 mol/L (9.90 - 27.80 nmol/L)
Serum SHBG 9 nmol/L (14.00 - 71.00 nmol/L)
Serum Prolactin level 519 mIU/L (73.00 - 407.00 mIU/L)
Serum Oestradiol level <37 pmol/L (40.00 - 162.00 pmol/L)
These were normal;
LH 2 IU/L (1 - 12 IU/L)
FSH 5 IU/L (1 - 12 IU/L)
Thyroid is also normal and Hashimotos is ruled out. The Gp did not check for Free T but my first tests showed that is was literally borderline low and out of range low.
What should I expect from the endo? Thank you for any replies.
You may struggle with TRT do to very low SHBG, you should inject testosterone every day or you may never feel anything. You should be inject a smaller dose than most, otherwise you will have to deal with a ton of Free T and Free E2. Do not go above 500 ng/dL!
Normal isn’t optimal, please share your normal thyroid labs, you can still have a dysfunctional thyroid within the normal ranges that most doctors miss completely. Unless you’re working with the best you don’t stand a chance at recovery.
Low SHBG is a known cause of low thyroid hormones. Low SHBG = anxiety and depression. It’s the reason why those with hypothyroidism have emotional problems.
Serum 5.1nmol Testosterone is U.K Values i am from U.K and that equals in U.S Blood test values 147 ng/dl to me that is LIFE Threatening well it’s basically dangerous in a sense to your Bones, Cardiovascular in long-term if you are indeed from U.K like i am brother, you should go the MENS health Clinic in Dorset,Poole England, they will definitely accept you based on your Extremely LOW Level of T 147. Men’s Health Clinic however isn’t Overall cheap i think Initial Cost can be over $600 dolars but afterwards better cost. However they will Supply you Test Enanthate / HCG and that will work for you i am certain with your Low FSH LH sounds like Secondary. Well then you really don’t have a complete picture, you may be normal, not optimal and TRT requires optimal thyroid function to work effectively. Some people need Free T3 at the top of the ranges to feel good, doctors don’t treat normal, you may have to pay out of pocket for treatment.
That’s a pretty big jump (.82 - 1.77) in TSH, that’s what I would expect to see with intermittent thyroid problems. Your first mistake is trusting a GP with hormones testing, they aren’t hormone doctors. Free T3 needs to be midrange or better.
SystemLord what do you think Brother of his 5.1Nmol U.S 147ng/dl it seems drastically low, Dangerous level to be having for more than 6 months or 1 year even, lower the T Lower the Estrogen = WRECKING Havoc on Bone Health & Cartilages. Not to scare him but is true. Could cause Arthritis if left untreated. He could take Animal Thyroid extract or that T3 supplement this other gentleman was on.
What do I think about 147 ng/dL, well I got to experience what it’s like to be 120 ng/dL and it wasn’t fun. Men who score below 300 ng/dL have a higher mortality rate, heart disease, cardiovascular disease, dementia and alzheimer’s for starters.
Low estrogen has other consequences, osteoporosis. Your mortality rate is even higher at those lower numbers, you won’t be walking around long at those levels, your veins are most likely thin and not transporting blood throughout your body. After I went on TRT veins started popping out showing through the skin, the way it was before I have problems with low T.
Do you have high blood pressure and high heart rate? I did, but not after starting TRT.
Endo’s within the NHS are a nightmare and are some of the dumbest doctors on earth, the stories I’ve heard are just leave me astonish! You may need to go private and pay out of pocket.

He definitely needs Men Health clinic. but also he needs to get his Thyroid Regulated. he Could take Organic Kelp for (Iodine). Animal Thyroid extract from New zealand Bovine. and then he can keep on them both, KELP iodine daily. and make sure his Free T3 like you said is in check as well as TSH it self. But personally i know people who went to Mens Health Clinic like i said to him earlier & got their Test & HCG immediately because it’s all Privately done but an extra cost. Which is Priceless for your Future & life.
Thats the thing, my GP couldn’t believe my results. He commented that I didn’t look like I had low testosterone as I didn’t have the physical features. I also thought I had at least a normal testosterone level as I have quite a big build for not going to the gym.
Being a student really doesn’t let me go private haha, would love to though if it would help me finally solve what I’ve been feeling for so long.
Before my Testicle Injury which left with me Tertiary hypogonadism, Which is a MIX between Primary & Secondary. It’s quite RARE but i finally on T.R.T now with HCG. i had 14 inch Biceps still. and yet just because i got 14 inch Biceps doesn’t look like i have Low Testosterone too by the way i am 32 years old. born in 1985. this happened when i was 30 yrs old in 2016. but i had to go on T.R.T as i was experiencing tons of BONE pains & Muscle loss (Wastage atrophy of muscle from being Low) Now i am feeling somewhat better but i only got on it, 2 weeks ago. and Happy i won’t look back.
When if you ever decide to visit that Endo because of if you cannot afford the Private High costs as u mentioned your a student. Tell your Endo, you want to Trial Sustanon 250 and show him your blood results Also MILK your symptoms if you feel have too, Mention got Zero erections and haven’t had sex in 10 months or something, being only 147ng/dl at 20 years of age i doubt they would refuse, Endo’s hormone DR would definitely be more open-minded than a regular MD GP doc.
SytemLord said inject everyday or you won’t feel anything i do agree, Sustanon has 4 Different test esters everyday might be Okay without crashes if you split a 150mg dose into 4 times per week. Monday, Wednesday, Friday & Sunday. 150 into 4 = 37.5mg per testosterone per injection on those days. that should boost ur SHBG yes.
My SHBG before TRT was 34 nmol/L on a scale of 16-55 NMOL so in Mid 30’s which is good for a SEX drive. i have no idea what my SHBG is now, since i took that blood test last year September 2017. I feel if you get into mid 20’s SHBG to mid 30’s you will have a much better sex drive, as it’s a sex specific hormone. long as your Estrogen is kept in line too brother.
One more thing brother KC1997 your prolactin was High by 100 plus Serum Prolactin level 519 mIU/L (73.00 - 407.00 mIU/L) you had 519 over 407 Lab Result, Higher Prolactin does kill Libido and i can imagine you feel not much Libido at all at the moment. you could take Cabergoline drug which will Lower Prolactin Cabergoline will. If you prefer natural route there may" exist an herb" to lower prolactin. but i heard lots of men had success with Cabergoline. I would probably only resort to that if it got even more Higher though.
