Female. High Total T, Normal to High Free T on Replacement

My wife 45 has been in T replacement for almost 3 years. When she first began 20mg t.cypionate IM weekly.
Her free T was at 23
Total T at 150

She felt great. Sex life off the charts, bonus for me.

She lowered amount to 10mg weekly and Free T dropped to 10 felt ok but not as good while at 20mg weekly. This went on for about a year. Then last year she went back to 20mg weekly IM and levels never got above 10 pg/ml. Her total T slowly began climbing up to 350 to 500 levels.

SHBG. 49. Range: 24-122
DHT Free. 0.50pg/ml. Range: 0.09-1.02
DHT 6.3ng/dl. Range 4-22

Any reason why Free T stayed hovering around
8-10 pg/ml but total T increased?

The only thing I can think of is her doc started her on low dose dutasteride 2x a week for genetically thin hair which her mother has.
Any other reasons why?

I would get her off this stuff because it’s going to affect other hormonal pathways because it’s an endocrine disruptor.

I agree with @systemlord on this 100%. For a women with thinning hair and genetic predisposition you likely shouldn’t supplement with T at all. If its necessary for well being, which I don’t think it is in most women, you should stick to the low end. 20mg/wk is a lot for a woman.

If this were a guy I would tell you the same thing, stop the dutasteride. DHT is needed for brain and sexual health.

My guess is that SHBG has gone up, although donā€˜T know what it was before, and women typically have little higher than men. But the test injections would lower it some
