Failed PCT - Getting Off TRT for Fertility Purposes

Do you know what your SA was before TRT? I suspect it would have been closer to what you have now.

Regardless, from experience, I doubt getting off TRT will help. As long as you take hmg and hcg you are giving your body the same signals that it would get if you were off ( so LH and FSH )

Unfortunately a SA was not done prior. You don’t think my numbers will improve upon getting off? HMG is extremely hard to get for me and clomid isn’t an option unfortunately.

I did a SA before and after being on hcg for more than a year my numbers didn’t change.

As for clomid, you can find it online pretty easily but I would really suggest going for HMG.

Was the SA done whilst you were on Test? Did you at any point get off test and get retested? I’m banking on the fact that testosterone is suppressing me despite being on HCG. I’m not totally infertile, my numbers were:

To the OP. I’ll chime in as well on this one. I never did a analysis prior. But got my wife pregnant in the first shot before going on T, coincidence. Probably not but maybe. Most docs i take to say coming off is most viable option at this point.
I think I may start the restart protocol of HCG and some form of FSH if possible.

Ill break down my experience chronologically so its easier to read:

Before getting on TRT ever : 21 million
During TRT and no HCG - 1 million
After 6 months of stopping TRT : 23 million
1.5 years after restarting TRT + 600 iu hcg per week - 23 million
4 months after dropping hcg to 300iu / week while still ok TRT - 3 million
Now ( after 6 months of being on hcg 900 iu / week + hmg 75 iu 3x /week ) : 20 million

Keep if mind, sperm concentrations depend a lot on ITT ( intra testicular testosterone ). So if were low T to start with, most probably your ITT was low too. So there is a possibility your sperm concentration was low already before TRT.

Curious, what was your natural total T and free T before TRT?

Thanks for clarifying. Judging by what I’ve been reading the past week, I must have had a low baseline prior to getting on T or for what ever reason, my body did not respond to HCG. Before getting on my levels were around 10.8/nmol.

When you did the 6 months off T, we’re you running any HCG or was that a clean 6 months off everything and then you get tested?

Also your sperm count, is that total ml or is that concentrate?

Yeah, I also think it must have been a condition pre TRT due to the low ITT during many years.

It was a clean break off, 6 months off everything.

Mine was concentration per ml. So total would be around 3.5x that number.

I’m going to PCT, do a clean break off everything and retest. On the bright side, it’ not azoospermia where we have zero sperm. I have an appointment with the fertility clinic in 2 weeks and have him look at my numbers. Based on the WHO’S guideline, I guess I’m close to range on everything except sperm concentration which (scored 12 where reference is between 15–259 million per ml).

Yeah your concentration is not that bad and it’s definitely workable.

Before going for a PCT, which would mean you need at least 6 months to see if it makes a difference, I would talk to the best fertility doctor you can find first.

6 months is not a huge amount of time but on the other hand you might feel miserable especially the first two months. So if you are going to suffer it must be worth it.

I have a feeling hcg + hmg might bring you to 15-20 million. Maybe try that first for 3 months. It’s extremely unlikely that with a pct you will jump to let’s say 30 million or more.

Anyway, as I said before, Im not a doctor and my opinion is based just from my experience. Finding a great fertility doctor would be your best bet.

Appreciate everything man! Really cant thank you enough. I’ve also seen a lot of folks have a ton of success by getting off and blasting HCG and HMG so we’ll see if it comes to that.

Fertility clinic I called said to drop the test, we’ll see what they say 2 weeks from now. I guess from what I’m reading, all hope is not lost with a natural pregnancy, just need to be more frequent based on my sperm concentration and volume.
