Hey everyone, looking for input from people with experience on HCG monotherapy or similar protocols.
Background: 33 year old male, healthy, fit, good diet, exercise regularly, sauna. No prior ED issues, could get and maintain erections for sex without any issues before starting.
Protocol: HCG monotherapy, started a few months ago. Original goal was improving mood and libido which were low. Have tried different doses, currently on 3 x 500 IU per week.
Bloodwork at trough (just before injection):

Current testosterone: 19 nmol/l / 548 ng/dl
- Baseline testosterone: 13 nmol/l / 375 ng/dl

Current estradiol: 136 pmol/l / 37 pg/ml
- Baseline estradiol: ~50 pmol/l / ~14 pg/ml (nearly 3x increase)

SHBG: 25 nmol/l

Prolactin: not yet tested
Current issues since starting HCG:

Unable to maintain erections for sex despite mental desire (tried with different girls, told them I had issues, they were really understanding so the anxiety I had left disappeared after that. Still having issues though.

Tadalafil (15mg) largely ineffective (before HCG I could get insane hard on 2.5-5mg Cialis). Even Viagra didnt do anything last time. Like, nothing.

Precum present without erection during arousal — never experienced this before HCG

Morning wood weak and only partial (50%) only there with tadalafil assistance

Original goals of improved mood and libido not achieved, only achieved more energy and muscle mass

Key observations:

Good circulation, healthy lifestyle rules out vascular causes

Seeing endocrinologist Monday, but I don’t have mucht faith in his expertise. He only looks at the testosterone value and said: you’re test is good so it’s mental.
This issue is ruining my dating life/potential for a relationship, even though the girls are very understanding. I’ve read a lot of stuff on T:E2 ratio, elevated estrogen levels, dampened dopamine response from the HCG etc. It’s a giant rabbit hole. I’m desperate, looking for some expertise and someone with experience.

So I don’t have any advice for you but I am in the EXACT same boat and I can’t find much when searching online, I was surprised to see your post cause there’s not much info on people having these types of issues with HCG. Now I’m on 400mg of test enanthate and also taking 50mg of proviron a day ( plus cialis as needed hahaha) so before starting the HCG I could quite literally hang wet jeans on my junk , my wife was legit scared of me lol …but then about 2-3 weeks into the HCG I started having issues, same as you with cialis having little to no effect on maintaining erections. I have run HCG in the past and it’s the best I have ever felt in my life , had that glow and that just natural feeling of “well being” all the time and boners were on point and always readily at attention lol , so I was very surprised when I started running the HCG now and I got none of those positive feelings I had before. Now I got my HCG from a domestic UGL Ultima Labs and they recently had some bad batches of test go out but they said no other compounds were affected and I have did a pregnancy test to confirm it was real so either it’s real HCG or I’m pregnant cause it was a positive test…well let me know if you find any info on this brother and I’ll make sure to do the same , I’m thinking maybe the fact that I was 27 when I ran the HCG before and now I’m 43. Oh and I was also running it at 500iu 3 x week .

It’s a real mystery. Did you do any bloodwork? Things like prolactin, estrogen etc?
The most interesting part for me it’s that even cialis and viagra dont work anymore, like its not that I got vascular issues suddenly. It must be the hormones (or mental), but I doubt that. Even at home I dont have much succes, let alone with woman. They can do whatever they want but that thing down there isn’t responding. Before this I could take a 5mg Cialis tablet and have fun the whole night, as much as possible. Last week I took 10MG cialis, even with small dose of Viagra, and it didn’t do shit. Can get erect with some effort, but lose it very quickly after that. Like there is no drive or urge to really go after it.
I’m tackling this from all angles (therapist, extensive bloodwork this week, appointment with the endocrinologist, getting iron infusions). But honestly I am close to stopping the HCG completely to see if I will start responding to sex and things like Viagra again.
Why did you start HCG when you had good libido and erections? Have you tried quitting the HCG to see if it come back?

Yeah no problem brother I’m glad I found somebody else with the same issue. I haven’t done bloods yet because my urologist sucks and couldn’t get another appt with him for a month from now. But I stopped the HCG about 2 weeks ago because I was pretty sure that was the cause and lo and behold the wife is terrified again lol , but yeah back to full mast dude. I was thinking the same thing like hey maybe it’s mental , it’s not my wife ( f’n smokeshow and anatomically equipped with the tightest v I’ve ever had the pleasure of destroying lol, plus not to mention we are just still insanely attracted to each other and have an amazing sex life ) so I was thinking maybe outside stress like work and all that stuff but I mean I’ve had all those stresses before and still been able to keep a hard on no problem. Yeah before I started the HCG I had no issues at all , I’m running the Proviron at 50mg ( 25 twice a day) and that alone I don’t even need cialis but I like how it gives me a little more girth lol , I was very blessed in the length department but am average in the girth side of things but yeah that’s the only reasons I take the cialis because I don’t technically have ED and have never had any issues staying hard until this run of HCG. It’s cool that the chicks you were with understood , I was feeling horrible because I didn’t want my wife thinking it was her but she knew it wasn’t because like I said we’ve always had an amazing sex life so it wasn’t very out of character for me. Yeah keep me posted buddy and I’ll let you know when I get my bloods done, let me know how it all shakes out for you. Also if you don’t mind me asking what brand was the HCG, Pharma( prescribed )or UGL lab?

Lmao that description of your wife and sex life hahah. Glad it’s all working again man. But your case is pretty obvious, if you stopped the HCG 2 weeks ago and now everything is working fine again, why even try HCG?
It also gives me hope for my case, if you recovered within 2 weeks. Hope I will experience the same. Can’t wait to pop the cialis again and have decent sex. And yes, didnt ever need it but the increased girth and firmness on cialis.. Well once you’ve tried it you can’t go back.
The chicks were very chill with it (surprised me a lot) and basically said they find cuddling and just being together even more important than having penetrative sex (or they were lying lmao, but they both want to see me again after that fiasco.. so maybe there’s a truth to it. ). One of them even said her ex had trouble for 8 months before they could have sex, and experienced it with 2 other guys as well. It’s so much more common than we think. We as males sometimes have the tendency to think we need to perform, have alpha male sex etc, most chicks don’t even think that way, only in porn scenes.
My HCG brand? It’s from a hospital on prescription, so 100% legit. It’s the brand Gonasi. Previously I had Brevactid. But on prescription the brand shouldnt really matter effect wise.
Will definitely keep you updated, expecting bloodwork in a few days. Not giving up on this, doctor sending me to a sex therapist without putting in any effort on the physical side is just unacceptable imo.
Sorry I forgot to answer the question about why I started the HCG , it was mainly because I had run it before with such positive results and I had been feeling kind of blah mentally lately and remembered how good I felt on the HCG before and Ultima was running a BOGO sale so I pulled the trigger. Only positive thing ( well two things actually) was that I got my balls back , went from acorns to f’n plums and I was busting off shots that would put porn stars to shame , I mean my wife was joking saying she was gonna start wearing a raincoat lol
