Advice after first 3 months of TRT

I’m 52 and I started TRT about 3 months ago. I inject once a week. They started me at .5mg. I feel slightly less depressed but I’m really emotional. Get teary eyed over ridiculous things and I’m concerned I’m starting to get a little bigger man boobs. Not ideal. I got blood work 3 days after my last injection. My total T started at 265. Here’s the results:

LH- .2
ESTRADOIL- 40
AST- 16
ALT- 14
HEMOGLOBIN- 15.2
HEMATOCRIT- 46
TOTAL T- 634

They are increasing my dosage to .75 mg/week and prescribing .5 mg Anastrozole Fi/week. Does this sound reasonable? Should I inject more frequently and hold off on the Anastrozole? I’m really hoping to just feel better. I’m so tired of having no motivation and being depressed.

First you’re not communicating properly. Your injection is a volume of liquid in ml not mg. Your dose concentration determines the mg. So clarify are you taking .5ml at 200mg/ml? Please post lab ranges. Your labs are not comprehensive enough to make decisions anyway. DO NOT increase your dose AND add anastrozole at the same time. That’s very unintelligent for a doctor to advise. One adjustment as a time.

You labs really need SHBG and FT before advising.

Personally, I wouldn’t increase the dose yet. And you’re just getting into kinda stable territory at 3 months but expect some ups and downs yet.

Don’t assume too much and again, you may want to hold off on adding more T or other compounds. Your estrogen isn’t ridiculous but may be higher than you ate used to which leads me to…

What’s your bodyfat percentage? You prone to gyno pre-TRT?

Sorry, my dosage was 100mg and they are raising it to 150mg. I don’t know why I wrote the volume. Right now , my BMI is 29.6 so I know I need to lose about 30 lbs. I have a little bit of a man boob situation, but it in the past, it’s gone away with weight loss. I started TRT a few weeks after my shoulder surgery. I gained about 15lbs after that sitting around eating garbage. I got up to 210 and I’m 5’8. I’ve been hitting the gym pretty hard the last few weeks and cut back on all the junk I was eating. I’m down to 195lbs. My goal is 165-170.

I’ve been on 100 mg weekly (split into two injections weekly) for almost a year. That has been my sweet spot.

I started at 120 mg but it was too much for my body to be TRT.

My main issue was: too much bodyfat. I’m much leaner and more stable. Now.

If you’re active loosing weight and new to TRT…yeah, you’re going to have some emotional roller-coaster. I would venture to say that’s somewhat normal.

I’d suggest staying the course but there are much more experienced members who may tell you otherwise.

I think I’m going to stay at 100 mg and just inject twice a week for another month and see how I feel. Hopefully I’ll be down another 10lbs by then.

Make sure you get better labs too. What I listed it a MUST to assess dosage.

I’ll order labs next month myself and I’ll make sure to add everything. They just order the bare minimum. Thanks.

Why? Your T value is normal. And your estradiol isn’t high either. What’s your body composition like? Your “man boobs” might be from just carrying too much fat.

You could

Likely what you should do.

Won’t fix a thing, if there even is a thing to take care of.

I didn’t read this after your OP before my previous response. Your “man boobs” are not the result of estradiol at 40.

I people vary, but my estradiol has been as high as 50-plus without any symptoms.

It’s just something I’m paranoid about. Glad to hear that.

I was still feeling like complete garbage, so I increased my dose from 50 mg 2x/week to 60 mg 2x/week. I did this for about a month. This was a problem. ED was ridiculous. Even urinating felt difficult. I got super emotional, I was going to just quit taking it all together. I got labs done and here were the results:

Albumin 4.2
SHBG 23
Total T 953
Estradoil 58
Prolactin 9.5
Free T 207
Bioavailable T 398.7

Seeing how high the E was, I decided to take the Anastrozole. The pills are .5mg so I cut them up into quarters, so each dose was roughly .12mg. I took the first dose of .12mg two days after my injection in the morning. By lunchtime I felt like a different person. Now I feel fucking amazing! It’s the first time in years that I feel normal. My anxiety is gone, my depression is gone, I even feel more confident talking to people. I had some decent social anxiety that had been slowly getting worse over the years. For the past two weeks I’ve been taking .25 of the AI with my 60mg of Test 2x/week. Since I started in February, I’ve lost almost 30lbs. The shoulder I had surgery on is better now than before the surgery. It’s about 90% of my other arm. I can see that I’m building muscle. My shirts are actually getting tight on my arms. I was a fat doughy mess for a long time. Still am, but I can see a huge improvement. It’s given me the drive to hit the gym harder than I have in years. I’m hoping as I drop more body fat, I can reduce the dosage and get rid of the AI all together. I’m going to stick with this for a while. I’m scheduled for blood work in about 2 months. If I continue to feel good, I’ll just wait until then. If not, I’ll get another test out of pocket.

Man, that’s great to hear. Hope the trend continues.

I’d want to see what your shgb is but I’m thinking it would be smart not to increase the dosage since it sounds like your estrogen is high. Splitting your injections into twice a week 50mg each would probably help the spikes and more level.
