35, Healthy, Strong, but .... Low T, Need Help

A SERM will have effects very fast. Because of the half life, it will take a week to build the final state serum levels of the drug.

Then LH/FSH will increase but by an amount that is individual to you. Then the testes need to respond to the increased LH/FSH and that tissue response has two phases. One response in immediate increase in T production, again limited by your individual characteristics. The second phase is increased size and firmness of the testes. This physical tissue response takes longer to happen and then the larger revitalized testes will produce more T.

If dose is too high, E2 can become a major problem because of excessive LH levels.

Fascinating… thanks for the info.

This AM I awoke to a glorious boner which lasted a pretty good while. …then decided to run and hide following some testicle pain. In doing some searching, I’ve found that’s not uncommon with Clomid. If as you say my testes are starting to produce more T and will eventually increase in size (lookout pants), maybe a little initial pain makes some sense.

Drugs take about 3-4 half lives to build up to close to steady state levels. The two main SERMs have metabolites with long half lives (about 14-33 days for the Clomid metabolite zuclomifene according to one study and about 14 days for the active metabolite desmethyl-Tamoxifen of Nolvadex), so after a week you are not even close to steady state.

For example, from http://www.druginfonet.com/tamoxfen.htm

“After initiation of therapy, steady state concentrations for tamoxifen are achieved in about 4 weeks and steady state concentrations for N-desmethyl tamoxifen are achieved in about 8 weeks, suggesting a half-life of approximately 14 days for this metabolite.”

So what you’re saying is, hold on to my butt, I’m just getting going.

Ok, one week in, here’s what I’ve noticed:

  1. I want to sleep more. A LOT more. I feel good during the day, but want to go to bed earlier and want to sleep in later. Feels like I’m sleeping well, just need MOAR of it.

  2. Balls are bigger and firmer.

  3. Libido increased a bit?

  4. Not much change in boner action. Feels like I’m getting hard a little easier and maybe being a little fuller, but Viagra had better effects on my erections. Still needing to use it for satisfying sex, but I’m taking less than before.

  5. Feels like I’m stronger in the gym. Made some gains. …but did a day of handheld video work and was WRECKED - my entire core and back was destroyed. Wonder if endurance has been negatively impacted, or if that’s just from being sick for the past few weeks.

  6. Acne. Got a couple zits, nothing too bad, one on my shoulder, one on my back. Not outside the statistical range for me, but suspicious.

Still early yet, I know - not bailing on the Clomid yet. Hitting the gym in the AM before work, so will update on whether muscle soreness is still around from Friday.

OK. Time for a much belated update.

Took the full course of Clomid. Due to the holidays, moving, and being sick, I took almost 2 months off from the gym. Been back in the past few weeks and have gotten my strength gains back. Endurance seems to be fine, and I still look pretty good too. Got a couple more pounds of fat to ditch before I look as good as I did when I stopped but I’m not far off.

The Clomid was interesting. The sleepiness continued until about a week after I was off - whereupon my sleep cycle became more normal, wanting to go to bed around 11PM and wake around 6:30 or 7 in the morning. I don’t wake much during the night and my overall stress and anxiety seem to be quite well controlled - which is interesting because my personal life has gotten more complicated - finances have gotten worse with my divorce. Acne did not persist. Never felt like a beast or anything, never saw any noticeable increase in ejaculate quantity - was a little disappointed by this, but as the GF pointed out, there was plenty to begin with.

Having been off it for a bit, I’ve noticed my sex drive is a little higher than it was before, set a new adult record for myself last month during a ridiculous weekend filled with practically non-stop sex. As with before, I can get hard without assistance, but I can’t seem to stay hard. Have not had any failures to launch with Viagra since the Clomid - sadly I think I will be wholly dependent on the Viagra from here on out unless I can figure out what’s going on with that, as it doesn’t appear to be hormonal.

Just recently got new test results:

Total Testosterone: 410 ng/dL (240 - 871 ng/dL)
FSH: 2.8 mIU/mL (no stated range)
LH: 2.1 mIU/mL (no stated range)

Both my FSH and LH are lower than before, and my Total T is higher. No Total T this time unfortunately… and since he apparently didn’t order SHBG (or they didn’t share that with me), I can’t calculate it. All told though, things look OK to me.
