33 on TRT, Looking for Options

So where to start. 5 yrs ago move into new house. Long story short get very sick from mold in house at this point i was working out 5 days a week 5’11 175lbs 7% body fat. After getting sick i start having panic attacks and anxiety(especially when physically active or when its hot outside)and physical exhaustion, exercise becomes impossible. Naturopath treats mold but still no energy, muscles weak, gaining weight even though eating super clean and used to have good metabolism and sex drive is dead and gone. Test testosterone levels, find free and total both extremely low. Like 130 and 4.

Went to specialist he starts me on trt injections. Been on them now for 2-3yrs. Help my energy a bit but still no where close to normal, sex drive is better. Still cant work out though, if I do a small workout the next day my muscles aren’t sore but weak. Like walking I feel like my legs will give out kinda weak. My red blood cell count always gets too high so specialist sends me for a phlebotomy every 3 months. But I don’t feel like this is working. I want to come off it or transition to Clomid as I hear red blood cell issues are less common.

But even that i dont feel very optimistic about as my levels are decent now bit i still dont feel great. I just feel like im wasting my life not feeling good. I would love to be off completely if i could somehow restart my own system but not sure how to try or go about it. Any help or suggestions or advice would be greatly appreciated!

Ps after reading through posts here I realize Clomid may not work for me but I was never tested to see if it would the specialist offered both but recommended injections. How would I test to verify this?

What is your dose/protocol? E2 levels?

75mg/ week I found best for me if I split the dose and do 40mg saturday 35mg wednesday. E2 is at 78 pmol/L

You can’t really expect that much help without posting a full set of labs including thyroid. Lots of guys who have low T also have low thyroid function and the symptoms are very similar. Your E2 labs are at the bottom of the normal range, it might be too low for you as your testosterone dose is a low weekly dose.

We do not know your age.
Do you need to preserve your testes and fertility?

Lab ranges please…

E2=78 pmol/L is nice. T levels could be higher, but then FT–>E2 would also increase.

If you are not tolerant of higher T levels, the possible issues could be increased E2, low thyroid function or weak adrenals.

Please post all lab work, not just those hormones and include lab ranges.

Where are you located? Affects diagnostic and treatment options.

Low energy cause needs to be identified and TRT–>clomid is a dumb move.

Low energy causes:

  • low thyroid function - self eval via oral body temps - see below
  • iodine deficiency causing above
  • low cortisol, check AM Cortisol and do at 8AM or 1 hour after waking up
  • elevated rT3, reverse T3, stress is a major factor and chronic health issues such as mold could be a cause of that.

Evaluate your overall thyroid function by checking oral body temperatures as per the thyroid basics sticky. Thyroid hormone fT3 is what gets the job done and it regulates mitochondrial activity, the source of ATP which is the universal currency of cellular energy. This is part of the body’s temperature control loop. This can get messed up if you are iodine deficient. In many countries, you need to be using iodized salt. Other countries add iodine to dairy or bread.

I can be a bit abrupt in my replies and recommendations. I have a lot of ground to cover as this forum has become much more active in the last two years. I can’t follow threads that go deep over time. You need to respond to all of my points and requests as soon as possible before you fall off of my radar. The worse problems are guys who ignore issues re thyroid, body temperatures, history of iodized salt. Please do not piss people off saying that lab results are normal, we need lab number and ranges.
