33 Looking to Start TRT Soon

Stats:
-age: 33 Male
-height: 5’5"
-waist: 32 1/2
-weight: 165
-describe body and facial hair: hairy arms and legs, not hairy chest or facial hair, Male Pattern baldness with very very little left on the top
-describe where you carry fat and how changed: mostly around waist and legs, it took a while to get here ~8years after i stopped working out religiously. I have yet to do my BF%

-health conditions, symptoms [history]: fatigued most of the time, aching feet & ankles when waking up,
difficulty waking up regardless of how little/much sleep, some depression at times, mood swings

-Rx and OTC drugs, any hair loss drugs or prostate drugs ever : never

My only worry is the labs ordered by my Urologist didn’t have E2. I thought I requested for it when they were ordering the labs and asked if i wanted anything else added. Should I get retested?

-describe diet [some create substantial damage with starvation diets]: anything and everything, attempts to eat healthy at times but doesn’t last longer than 2-4 days,
some junk mixed (chips/burgers/fries/pizza/etc) with balanced meals (lean protein,brown/white/mixed rice,up to a cup of veggies at times)

-describe training [some ruin there hormones by over training]: none; aside from the occasional jog to the train station (.6 miles) once a day. I can do this in about 4 minutes in the Texas heat, but I’m sweating pretty bad and very winded by then. I usually do the jog after work wearing jeans, a t-shirt+dress shirt, and a backpack with work items (~7-10 lbs). 4 minutes is when i’m running late other wise i try to keep it at 5-6 minutes pacing myself. This has stopped for about 2 weeks since switching to driving my wife to work until she goes on medical leave (she’s pregnant with our second)

-testes ache, ever, with a fever? rarely and randomly will I have some aching on the left side, but i can’t tell where it ends or begins. It feels like it’s twisted and adjusting it doesn’t help. I just have to wait it out. never with fever.

-how have morning wood and nocturnal erections changed: definitely not as strong as they used to be, but still happen. I’m not fully erect in the morning, and I think I have pretty consistent nocturnal erections


I have a followup appointment with my Urologist Wed morning to go over these lab results and for my vasectomy consultation. The vasectomy will be scheduled roughly late December/early January. My labs came back today and I’m seeking your help help understanding these results and what I need to prepare for my visit. I understand using the previously mentioned stickies what these obvious hormones are (lh,fsh,shgb), but I can’t tell which is which for the Testosterone portions.

I’m lucky that my wife works here so I may have some freedom with the Doctors wanting to take extra care of me. They like to use pellets here, but also have the other forms of treatment as well. I do know that there have been cases at this practice where patients are approved for self injections, but the approved dosage is lower than being injected at the office? I’m going to clarify this. I’m not against pellets as almost all the male staff there has them injected regularly.

Your LH/FSH levels are low. Prolactin is not the cause. E2 unknown. ← yes get it tested

We do not know what is suppressing your LH/FSH.

Pain in testes: Get them examined and also check for a degree of a hernia.

No pellets please. Levels change over time and that makes AI [aromatase inhibitor] dosing a bit impossible.

Injection is the best delivery system [cost and steady levels]. You need to self inject.

Test TSH and check body temperatures as per the thyroid basics sticky. You have always used iodized salt. We frequently see thyroid issues in this forum.

You are overweight. Fixing T and T:E2 will allow you to loose fat, if thyroid levels are OK.
