24 Years Old with Low T

In the last 4 years I’ve put on about 90 + pounds and I’ve been feeling like sh*t! High b/p, fuzzy memory, little to no sex drive, hard time concentrating, weight gain, an aver all feeling of being depressed and not caring about one thing. Fits Low testosterone to a T!! In the beginning of May 2012 I went to the doctor and had some tests done.

Total T: 124 ng/dl
SHBG: 14.23 nmol/L
FSH: 2.7 mIU/mL
LH: 2.0 mIU/mL
PSA: .25 ng/dl
Free T4: 1.38 ng/dl
TSH 1.95 uIU/mL

There was also some others but they are not related to low T. I have not taken a free testosterone test, my dr has not ordered that yet. In the next couple days I will be doing a Cortisol test (24 hour urine sample), Somatomedin-c, prolactin, CBC no diff, and PSA. I’ll post thoughs when I get the results.

I had an MRI done and I have a benign neoplasm pituitary microadenoma. Which is a tumor in my pituitary gland. This is the reason for my hypogonadism.

My endocrinologist has prescribed me 200 mg of testosterone Cypionate every 2 weeks. So 100 mg a week. That is all. Then he will re test me in 3 months and see where my levels are at and I’ll have another MRI to make sure the tumor is not growing.

I asked my endo about an Aromatase inhibitor and he said no. Does anyone think I’ll need to take one, especially me being over weight and having a high body fat? They haven’t tested my estrogen levels, but the endo said the Cortisol test will tell them that…

Also, me being so young would anyone bump up the testosterone to 200 mg a week instead of 100 mg? After much research on t levels, I would like to be in the high normal range.

I also asked him about HCG and he said we will talk about that when you want kids. I’ve read people do a low dose everyday to keep there balls from not shrinking, aching, and still being able to produce. Is HCG a good idea? My uncle has secondary Hypogonadism and he is prescribed HCG everyday for the rest of his life. He uses an insulin needle and does 30 units in to his thigh.

This is my first post, hope no one hates on me. I’d love to know peoples opinions about what my endo put me on and also the AI, HCG and if I should bump up the dosage?

Any doctor that states estrogen levels can be inferred from cortisol should be drug out into the streets and shot

Im not saying your doctor IS an idiot, but he certainly is filling out the checklist

You need more frequent dosing. Your biweekly volume is fine, but split that up into 4 injections over that time (50 mg 2x/week). Don’t raise it before you figure out how you react at the standard level.

Truthfully, since you are secondary, you would be a candidate for hcg monotherapy like your uncle, IMO. You may want to pay a visit to his doc to see if you can be treated similarly?

Yeah I have no idea why he won’t check my estradiol levels.

The biggest reason I can’t go to my uncles Dr. is $$. I have wonderful kaiser (wink, wink) and have to use what they give me. But the test is very cheap.

I know where I can get an AI, would you recommend me getting one?

Also, would you reccomend me getting a blood test before 3 months? Wouldn’t it be better to re-check in a month? So then the Dr. could dial in my dosage, rather then leaving me in the dark for 3 months?

4-6 weeks would be ideal.

I don’t recommend you get an AI without first testing your estradiol to assess its need.

Get an E2 test done before you begin, and another one 6 weeks later. You can pay out of pocket for this at www.privatemdlabs.com

Sounds good. I’ll do that! Thanks for the feedback! I appreciate it!
