What would you make out of these labs/moving forward treatment wise?
No free testosterone was done, however, with the high SHBG we can assume it was low? I have no major improvements in symptoms even though my testosterone has increased but I assume it is because my high SHBG? Is there any way to decrease SHBG? Would switching to TRT be my only option?

Large doses of testosterone will lower SHBG, but how much is based on the individual. You might be able to get by with one large weekly dosage of testosterone or two largest doses. Excess androgens is what lowers SHBG, or rather the large peaks.

You just may require supraphysiological doses to hammer down the high SHBG. No major change in symptoms, expected with SHBG high. Too much testosterone is bound to SHBG. If you throw more HCG at the problem, it will only increase estrogen, not good.

Unfortunately, HCG/clomid and nolvadex all increase SHBG.

Are you taking any medications? Keto? Intermittent Fasting?

Propecia/ Finasteride? Accutane?

You probably don’t “feel” any better because your SHBG is pretty high, and your E2 went up from the HCG. So your probably in an estrogen dominant state right now.

In my experience, the doc gave me HCG monotherapy , along with Arimidex to make sure estrogen levels stay low. Put me on Arimidex 2x weekly, the entire time i was on hcg monotherapy.
