Trying to Find the Source of the Problem

Got my blood work back today after 4 weeks of 25mg clomid EOD…

Testosterone, Serum 756 (348-1197 ng/dl)
Estradiol 16.9 (7.6-42.6 pg/ml)
LH 9.3 (1.7-8.6 Miu/ml)
FSH 7.5 (1.5-12.4 miu/ml)

I’m thinking those are good numbers? I thought my estrogen would be higher and have adex on hand, which I haven’t used yet… I guess I’ll stay in 25 clomid EOD for afew more weeks and begin to taper off.

I feel decent, but not big dif in energy… libido is up some for sure.

Any thoughts or advice with my current lab work results?

are those good numbers for mid restart attempt?

Mood was ok on clomid and I felt fine, not a big difference but my libido was definitely up.

Fast Forward to now, I went to endo appointment and he checked afew things…

Prolactin 1.7^NG/ML (range 1.9-25.9) (which was expected as I’m on caber)
Thyroid Microsomal AntiBod .06^IU/ML (range 0.0-9.0) no idea what this is?
TSH .016^MIU/ML (range .34-5.60)
Free T4 1^NG/DL (range 0.61-1.24)

Ok I see my TSH is super low and he never said squat about it. Hes still not even going to test my test and e2 etc… This and when he told me he wants to to cut back on my caber to 1/2 a pill a week instead of 1/2 2x a week I decided to make an appointment with a new endo.
With the TSH being so low does that mean that I’m primary and will need TRT?

just noticed I asked if the low TSH was a sign of primary … I got it confused with FSH… anyway with TSH so low do I need to be on thyroid meds? what do I need to tell my new endo?

Was TSH written down wrong? Should have been flagged out of range low.
I would as for a retest or correction.

Low TSH is a sign of hyperthyroidism which shows up with weight/fat loss and higher body temperatures.

What are current body temps?
Have you increased iodine intake?
Did you do iodine replenishment at some point?

I had just gotten over a cold where my temps were 100-101 dunno if that caused it to be effed up.
I have increased iodine and about 6 months ago I did the ID protocol.
My temps have been under 97.5 in morning, but seem to be getting up to 98.6 during the day. I just checked at 4:00pm and it is 98.6

I think when I goto new endo later this month Ill just give him entire story about my low test concerns and the fact about my growth on pituitary along with nodules on my thyroid… hopefully the new guy will just run me through the ringer of tests , instead of ignoring low test. if he ignores it, I guess next step is finding a urologist

Thyroid nodules can depress TSH by creating high thyroid hormone levels that act on the negative feedback loop. fT4 is 1.0 and midrange is 0.925, that does not seem to fit the pattern, but we have no idea about fT3 [was low]. Body temps are a factor, you were getting to 98.6. Are you heat intolerant?

Could you also be seeing an enlarged thyroid?

I am not getting clear responses to:

  • your history of using iodized salt
  • current body temperatures

Trying to understand your thyroid nodules and possible hyperthyroidism. Iodine deficiency increases TSH and that overstimulates the thyroid leading to nodules. A lack of selenium in this case can lead to inflammation that can lead to things going wrong, nodules to cancer, take your pick. Iodine replenishment always needs an identifiable source of selenium. Multi-vits that list iodine+selenium are a good choice and for almost all men, you need one that does not list iron. Do not think that the thyroid nodules are not presenting something that is not serious.

My body temps have been getting up to 98.6 or just below (98.1-5)during the afternoon… My body temps in the AM seem to be low (97.3-7). I did Iodine replenishment and still am keeping iodine and selenium in my supplement stack at least 2x a week. I also have a multi that has both in it along with no iron.

As for the thyroid nodules. Apparently they are very small and my endo palpated my thyroid and told me that it is not enlarged and everything ‘felt’ normal.

I am fed up with my current endo and am scheduled with a new endo in May. I am going to be up front and tell him that I want my thyroid completely checked with complete blood work. I will also tell him of my concern of my hypogonadism and will let him now if he dosnt want to do TRT, I will just get it from my primary care Dr. I just want to know if Im primary or secondary hypogonad.

I have a feeling the only way for me to get my test back to normal will be TRT and if that’s the route I need to take, I will. Im just getting tired of this slow motion crap with these endos.

Your response to 4 months of clomid showed that your testes work, you are not primary. Your first labs suggested secondary.

With improved body temps, are you feeling better in terms of focus and clarity?

Endo’s as a group seem to be the worst at TRT and for regards of guys’ hormonal quality of life.

I have been checking my body temps for the past week. Most days I start off under 97 or right at 97 degrees. By Mid day I’m getting to 98.1 and later in the day I get close to 98.6 but not all the way there. I’m still supplementing both iodine and selenium. My focus and clarity has not improved.

I had my new Endo appointment on Monday. He looked at my previous levels and stated that they were in fact low for my age. He said he’s going to do a ultrasound on my thyroid for the nodules and check bloods on test, prolactin and thyroid.

FFW to yesterday I went to get blood work done and got results back (SAME DAY!).

It looks like he didn’t do a complete check on everything. I don’t understand why these Dr’s wouldn’t just do a complete panel just to see where everything is. my insurance covers it for crying out loud.

Anyway here is the results of the things he had checked. The ranges are different than my previous panels, so I hope he doesn’t go by the outdated lower ranges in deciding to place me on TRT.

TT 300 ng/dL (range 160-726) not surprised its low, but those ranges?
Free T 6.6 ng/dL (range 3.3-14.1) same wtf at the range?
% free test (calc)% value: 2.2 (range 1.5-4.2)
Free T4 1.2nd/dL (range 0.8-1.5)
TSH 0.52 (range 0.36-5.00)
Prolactin 2.5ng/mL (range 2.5-17.4)

He did not check estrogen and t3… have no idea why. I guess I will have to order bloods on that on my own. I’m waiting to hear back from him on the results and am hoping he places me on TRT as I am fed up with this low test crap. If/when he puts me on TRT should I mention HCG or an AI? Or should I just wait to see what my levels of everything is at? I don’t think I’ll have a problem with E2 because it always comes back in the low range and when I did the attempt at the restart with the clomid my test was up around 700 and the e2 never got high.

I’m not worried about being fertile because I’m getting a vasectomy anyway soon (3 kids. my only reason for the HCG would be to keep my nuts normal sized.

fT3 is the active hormone, not testing that seems really bogus to me, but I am only an engineer who looks at how things work or do not.

Your body temperatures are determined by fT3 levels that increase body temperatures by upregulation of mitochondrial metabolism and rT3 can interfere with that.

Lower TSH can be indicating might be from a thyroid nodule. Those can be promoted by higher TSH + years, often from low iodine intake. Hypo can progress to hyper. See what ultrasound indicates.

This is getting ridiculous… I just received a letter from my new endo that stated that my testosterone levels are within normal range (refer to bloods listed above). I guess its true that endos are worthless when it comes to male hormones.
I just sent the endo an email basically telling them that I don’t feel like they are in range and all my previous tests had different ranges and I’m well below those. I also added that my current test levels are that of a 80 year old male despite the fact that Im 32 years old in June.
I ended the email stating that I would prefer my TRT be administered by an endo, but if he is not willing to do the treatment to please send the lab results to my primary care physician who WILL place me on TRT, even though she is clueless about it (didn’t put that in email).
Oh well, looks like I’ll be using my primary care physician and just hope she will listen to me about HCG and watching e2 levels etc…

Ok … with much surprise the endo has wrote me a Rx for 100mg test-c per week! I plan on getting insulin syringes and injecting SC 2x a week.
He has me scheduled for follow up blood panel in 6 weeks… do I need to do my own lab work before than? Or is the 6 week mark an ok amount of time from first injection?

Just picked up my test-c from Walmart pharmacy… they are 2 tiny 200ml vials… is this just a Walmart thing or did my dr write rx for small vials?

Just did my 2nd injection of test at 50mg…, today my face has been red and flush along with hot feeling… I’ve seen where it could be from high e2… my next lab appointment is not until end of June… should I go on my own and get an e2 test or give it afew weeks and see if it evens out?

Forgot to add that I have armidex (not from dr) should I go ahead and use it or would it be wise to wait for labs so dr will possibly prescribe it?

Just wondering with this being my first week of trt and 2nd injection is this normal

I’ve been doing 50mg test c 2x a week (Monday PM injection Friday AM injection)Got my 6 week labs today (Thursday) and got my total test and free t levels back today. I have not received results for my SBHG, e2 or DHT yet.
With that being said here are the results thus far:
Total test 457ng/dl (range 129-767)
Free test 11ng/dl (range 3.3-14.1)
% free test 2.4(1.5-4.2)

Also got free t3 tested and it came back as:
3.2pg/ml (range 2.2-4.0)

I was hoping that 100mg test c a week would get me to much higher than 457 the day before my next injection. Should I bump up my dosage to 150mg a week split up in two doses? Or do more frequent injections possibly every 3 days rather than 3.5? I’m thinking it’s going to take a higher dosage to get my test numbers up. What say you?

I’ve been feeling tired a lot and not much energy. My sex drive has increased a little. I was concerned about free test being 11 even though it shows in the top end of the range of my lab, because I have seen other lab ranges of labs that others have posted and the 11 would be low on those lab ranges. Anyway, I got my other labs back today and they are as follow:

With my estradiol being 40 the day before my next injection, should I start .25 armidex the days of my injections (2x a week)?

I’m thinking about bumping up test C to 120/week and adding AI at .25 2x week. I have AI on hand and I believe that the estradiol being in the 40’s (it was under 5 pre TRT) is causing me to be tired with no energy. I have felt a flush in my face from it and itchy nipples. I also believe I am holding water and look moon faced/bloated.
