Your Free T3 took a nosedive, was 4.1 and is now 2.7, one is optimal and the other suboptimal, you need consent thyroid hormone to feel good.
He is on iodine. I thought it was known to increase tsh. I would stop the iodine.
I missed that little detail, but still his Free T3 is suboptimal, some need Free T3 high normal to feel good. Also as we age our receptors become resistant to hormones and need more to feel normal, not everyone, but some of us.
If you feel lots of fatigue. Hard to wake up and focus let me know. This is thyroid usually. For me it was. A properly functioning thyroid should come first and the testosterone will only work when that’s fixed.
It’s like the fatigue from thyroid overrides anything beneficial from the T.
My TSH goes up to 4 while on TRT. If I stop it goes right back down. I wouldn’t get too worked up about that number short term. Looking over the post, I think his protocol was just too low of a dose. Total T was mediocre and E2 was quite low. Switching to daily injections or whatever nonsense isn’t going to fix that. Sure, he had low SHBG, but that seems pretty irrelevant given the other numbers.
My TSH was on a downward trend to from 1.7 a two years ago to the previous test. This one was kind of a kick in the teeth.
@enackers brain fog and fatigue has been the main issue for years when I started TRT and they did an arse load of tests and a contrast something another for pituitary and thyroid I believe that was about 4-5 years ago now.
His Free T was optimal when he was doing 40mg every 2 days, his Free T was 3.2 % slightly above normal. Remember his SHBG is low so he will need less Total T value than most men to have optimal Free T.
While his E2 was low on paper, nobody considered his Free E2, similar to how his Total T would appears suboptimal, then you see his free hormones are plentiful.
His free T was a very nice number, but apparently not optimal for him, or at least not optimal enough. His other numbers kind of sucked.
I would say everything from mood to fatigue to brain fog were fixed with thyroid. That’s when TRT started to amplify those effects.
I get what you are saying and sure if it goes up an down with Trt it’s having a logical cause, but we should never compare in the world of hot. Anyone that has Tsh increase to a abnormal level should have their thyroid checked for malfunction. If it doesn’t have a problem so be it, but more than likely the TSH is indicating t)Myers is a problem.
I would like to see if TRT increases tsh because you have an abundance of Hormone and it’s workinf harder with this surplus. This could be why tsh goes up with trt and then drops when there is not much hormone to manage.
I’m being curious here not saying I know for a fact or you are neccisarily wrong. I would love to see if you did a lab test for thyroid before, during and when you came off. It would answer questions.

I have one from last month somewhere where I was off, it’s just above 2 I believe. I am back on,and will probably get that checked at some point while back on.
Ok but need to check the free t3 and etc when tsh drops and increases. That will show what’s actually happening .
Free T3 for TSH 4.0 is 2.9 range 2.1 - 4.7
Free T4 for TSH 4.0 is 1.2 range 0.8 - 2.0
Total T3 1.1 range 0.7 - 2.0
Total T4 6.5 range 4.5 - 14.0
Don’t have those numbers for the other two. Probably have those numbers for the one last month if I can find it.
T4 should be midrange, you may have been meant to operate (Free T3) in the high normal ranges and is likely why TSH is elevated.
These aren’t the optimal ranges, but suboptimal.
So general consensus is I need to figure out what the hell is going on with my thyroid
Yah I would. It’s a safe bet and worth the energy to ivrstigate. Just make sure you check it properly. Don’t run off tsh ranges. If the range is 1-5 they and they’ll say your fine at 4.5, but if you go to 5.2 they say it’s a problem … mind boggling nonsense …
Check the stickies and get the right labs.
Just did a consult looks like I’ll be adding T3 5mcg and stair step weekly to 25mcg to find a happy balance altho I’m a bit skeptical doesn’t this stuff have a short half life that likes to be split to twice daily doses? I’m going to be at once daily AM doses.
Yes it does . About 8 hours. I take it twice and most good docs have patients take twice daily.
If take it once I feel like total shit when I wake up.
I’d it’s a extended release maybe different and I take half grain am and Pm.
Just take it twice daily if it’s not some type of extended release tablet.
You might notice you feel more energy and well being hours after first and wake up feeling worse.
If that’s the case take it in am and again 8-12
Hours later. Just don’t take it to close to bed otherwise you might not sleep well for the first few days or weeks until the body normalizes.
These are a demonstration of the thyroid and related systems functioning correctly, actually. TSH is up in response to other levels, as it should be. Sub-optimal would be if the TSH was still at 1 or something like that. Your body should adjust to cope with changes, it’s never going to be all “optimal” numbers for anyone actually living a life. I was just pointing out that a jump in numbers when you change something, like starting TRT, is not necessarily any cause for concern in the short term, but reassuring that something is working correctly.
