New to TRT, Dosage Question

It’s higher than I recommend for a starting does, but it’s also not outrageous. I do like that they split the dose into 2. I usually recommend starting lower and working your way up to a dose where symptoms are abated and/or Free T is in the high end of the normal range.

Keep in mind that it’s not just T levels that you are treating. The T initiates a cascade of neuroendocrine changes in the body and it takes time for those changes to stabilize into a new balance. It’s at that point you evaluate how well (or not) the treatment is working. it usually takes about 4-6 weeks for this to occur.

If it were me starting all over, I’d start at about 52.5mg (0.25 mL) twice per week for 4 weeks then evaluate (labs would be nice too). if symptoms persist, increase to 63 mg (0.3 mL) twice per week for 4 weeks then evaluate. My guess is that’s going to be close to optimal, but you may wish to bump it up to 0.35 mL to see how you like it.

Here are the results of a recent dose-response study I did on myself to see what the Free T levels result from different doses. I’m currently at 140 mg/week in an every 2 day protocol and I feel great and am getting good results in the gym. I have noticed that I don’t sleep as well as at the lower doses and I have a bit more anxiety, so I plan to drop it down to 125 mg next week and evaluate. I think that may end up being my new standard ‘optimized’ dose.

Hello I am new to trt, I am a 28 year old male with the test levels of 466 last time I had it checked. I have my trt throught trt nation which does telemedicine. I want to know if the dose they are starting me off is way too high. It seems to me like it is. Here is the dosage below they want to have me start taking.

on the bottle it says 0.4ml twice a week. 1ml is 210mg, so they have me on 168mg’s a week if my math is correct. Isn’t this too much?

No. It really depends on your bloods next time they check (3 months?). What works for once may not work for another.

Have you looked into reasons your T might be low in the first place? At your age it’s not uncommon to have an underlying issue that is correctable.

Why 210mg per vial? Is it a testosterone blend?

Excellent question! Particularly the compounded blend part. If it’s a blend, the dynamics of T release will be different that with pure T-cyp and depends on the components and ratios of the various esters.

IMO, since you are still overweight, and took the test at 1 PM and ended up with 466 ng/dL and 20 for Free T, I think you need to think about it more. By 1 PM you might have dropped 20%. That would get you to 580 ng/dL. If you loose more weight, that might get you to good levels naturally.

466 when levels range from 300 to 900 ? Would you buy a corvette that said 0-60 and went 30 but because it goes the speed limit and could get u where u need to go ?

That 210mg/ml is quite curious. Never seen a bottle like that. In the US, at least, you get Cyp at either 100 or 200 mg/ml. Elsewhere it’s usually either enanthate at the same strengths…or Sustanon usually at 250/ml.

Note that the mg/ml is the dosing of the compounds (which is how your prescription is written). However, the testosterone your body cares about is something less than that. TCyp, for example, is about 68% T.

Compounding pharmacies will do that sometimes. It is my understanding using cheaper oils impacts the bioavailability of testosterone, so they put extra testosterone into the blend to make up for it.

There is a pharma brand that I can’t remember the name of that is prop mixed with test E or C that comes in this strength. I was thinking it’s in Australia but it’s probably Eastern europe.

The test is test cyp. Also sorry for not responding sooner to everyone being a new poster it only let me make 20 posts for my first day and then gave me a 20 hour cool down. Thanks for everyone’s input. For the people asking me why my test is low, I’m not quite sure, I decided to wait to start trt and drop down to at least 15% bf first and see where my levels are at there.

Mine is 220mg/ml. Compounded 200mg Cypionate + 20mg Enanthate. It works. They call it “Testosterone Cypionate Plus”. It’s probably just a selling point a clinic uses.

It’s a bit more complicated that it first appears, you would expect to see a man with a TT 466 with low SHBG (15) with a similar Free T when compared to a guy with an TT 800 with an SHBG at 35. Normally I see guys complaining of low-T symptoms

The Total T is inactive and is bound to SHBG, the higher the SHBG, the lower the Free T. I remember a member with SHBG well over 100 and his Total T was 1000> and his Free T was extremely low, lower than I typically see around these TRT boards.

The Free T is active hormone and yours is in the high normal range pretreatment. Your symptoms could be cause by 100 different medical conditions so don’t be surprised if TRT doesn’t get rid of your symptoms.

If this occurs you will know your testosterone was fine.

Impossible to say for sure. My hairline has receded more on T. Not to the point of baldness. My acne has been not so great. I learned you can try to control it just like you would have tried back then, with OTC acne treatments like Stridex pads. They work and I wish I had done it sooner.

OTOH, my balls have not shrunk at all where many complain about theirs shrinking.

How you respond is very individual. No point in worrying over it, as the things I worried about (shrinking balls) aren’t the things that ended up happening. If the TRT is needed, the quality of life gain is worth a lot of the negative effects.
