Thatās a solid starting dose. However, Iām not convinced low T is your problem.
I usually give three major recommendations to guys before starting TRT for the rest of their life.
- Get all your hormones checked (you did that)
- Try an elimination diet
- Get a sleep study
Good sleep is the single most important thing you can do for your body next to diet. Without it you can have any number of problems. Regarding diet, you could have issues with certain foods, pretty much everyone does. Figure out what those foods are and get rid of them.
Thyroid caused very similar issues for me in terms of depression and fatigue. No matter how much I slept, I never felt rested , and I could sleep more.
I had similar TSH levels as you and thyroid + TRT (which you need) helped a ton. It wasnāt until started thyroid did my fatigue and depression vanish.
These doctors will say anything in range is ok and you are healthy. Iām sure you realize that is BS.
By the way my father is 70s and his free t is 15 (I think our labs max out at 21 right now) and total is 550/600 out of 1200.
You should contact balance my hormones anyways: they operate on that part of the world and might be able to help. You never knowā¦

I did diet elimination three years ago. I cut out gluten, diary, sugar, alcohol etc and it did not help at all. I have not done sleep study yet but I have a poor sleep. I wake up a lot during night and when I wake up in the morning I feel anxious and depressed, mind full of negative thoughts.
My TSH and FT4 are ok. What should I do next regarding my thyroid ?
I would give TRT a good run and if fatigue does not dissipate you will eventually want to give thyroid a trial run. I wouldnt be surprised if your psych pills are needed due to your hormones being low. Without hormones we have all types of these issues.
But and from personal experience it sounds like you are having a fatigue and other issues that come with low thyroid. I could be wrong, but its a solid reason.
T does not cause the same type of fatigue as Thyroid. Thyroid is the type of fatigue where your body never feels rest. You can sleep 12 hours, wake up , and want to slep more. You can sleep 10 hours go to work and by lunch you are so exhausted you fall asleep. This includes depression , anxiety as well.
Your TSH and ft4 are not ok if you have symptoms correlation to low thyroid. like i said my thyroid TSH was similar to yours. t4 doesnāt tell us much, because it is used to produce free t3 , which is the thyroid hormone that produces the beneficial effects in the body from energy to organic function, body temps and soooo much more.
I can have a TSH of 5 and my free t3 can be perfect with zero symptoms. Another person like me can have a TSH of 2 and my free t3 can be super low.
The west uses TSH as an indicator now, but they used to go with other indicators. Your doctor will say your TSH is in range, regardless of symptoms. Its unfortunate.
Here is the basic solution for men with hormone issues.

Thyroid for men isnt as bad as T, but its a noticeable issue and can cause many of the issues i outlined above. You have very low energy, this sounds like a thyroid issue to me.
I am not 100% of anything because i am not you. My responses are suggestions based on my experience and education my doctor has shared with me over the years.
Just remember to keep it simple and donāt over complicate it.

No clue. The easiest way to get TRT to produce benefits is by dose and symptom resolution. Start with a comfortable dose (individual preference) and increase until you feel like its working. Its a very subtle feeling when it starts working, but youll know it is working. It takes a long time, so one has to be patient. 3-4 months can be the start of benefits for some, and around 1-2 months folks notice a boost in libido. Problem is that your thyroid needs work. Without thyroid you wonāt feel the TRT working.

for anxiety, and by proxy sleep, this is what I did. My issue was chronically high adrenaline. Tired but wired. What most people consider an anxiety attack was a normal day for me. Really only went away when I had a few cocktails. Every Monday morning felt like a full-on panic attack.
Progesterone cream - 1 pump (50mg) 3-4x daily. Morning, lunch, dinner, before bed. Search Dr. Michael Platt for more info.
Glycine powder - 5-10g 3x daily. Usually 2x daily now. 10g after work and 10g before bed.
Z-12, Magnesium and time release melatonin before bed.
I think Iāll always have some form of anxiety, but it is very muted. I still have the thoughts, just donāt feel like shit all the time. Also got off sleep meds.
