You started too heavy. I would think about dropping your max too.
It’s better to be hitting rep PR’s on your as many as possible sets, then just hitting the requirements. Some days you will be ‘off’ and just hit the prescribed reps, but getting 5 & 6’s on your 3RM day is better than grinding out 3 and that being hard as hell to get the third.
The way I see it, and I could be wrong, I’d rather hit 3 or 4 on my 1RM day, and bail when form deteriorates rather than use shit form to grind out 1 or 2 just to say I hit it for two, ya know what I mean?

I have been doing 5/3/1 since Feb and this is the first time that I have missed. I went up 5 lbs like I am supposed to. So not sure what you mean buy dropping max. Do you mean repeat the last successful 4 week set instead of the unsuccessful 4 week set after my deload and then try to repeat the unsuccessful one after that?
Started too heavy? Do you mean that day? That lift was the last set that day, I had no problem with the 75% or 85% totals that day. Just the 95%. Same with 90% the week before and 85% the week before that. The final sets were the ones sucking.
And to Bro Chris yes I hit 95Lbs for 3 reps.

edit If I am getting video of bench form should it be with deload weight or wait until it is week one weight, so it is heavier?
