Generally, one can test for fasting response to glucose with a 3 hour glucose tolerance test. Sometimes, metformin can fix the issue. SHBG can also be suppressed by inflammation or NALFD. When it’s idiopathic, there’s no known cure, but I’m surprised your endocrinologist didn’t look straight into type II diabetes, as that’s the strongest link to SHBG that low.
