31 Y/O Ngiapmac's Journey to Solving Low T

Hey man, thanks for keeping up with your log. I know how frustrating it can be in Ontario to find good doctors, but you can request a second opinion. Do some research into finding someone you want to see (ratemds, google, etc) and ask for a referral. Either from the endo or your family doc or a walk-in. I don’t think they would or even can deny you a second opinion if you’re not satisfied. I just started seeing Dr. Komer for TRT and so far am very happy with him. If you’re looking to continue down the TRT path, I would recommend seeing him, but the waiting list is long.

Thanks for the feedback brother, this is actually helpful.

Are you referring to Dr. Larry Komer in Burlington? I don’t mind the long wait if he is going to provide great service. I also didn’t know that a walk-in clinic could refer me to him. I think that this is the route that I will take, as my family doctor is an idiot and hesitant to refer me to anyone. I had to fight with him just to see an endo, and in my latest appointment with him he wouldn’t refer me to a urologist.

I searched google for “blood tests in canada” and found a number of responses … perhaps you can try that provided you are willing to pay for your testing out of pocket.

I am aware that there are a couple of different options in Canada when it comes to getting bloodwork done. The only problem is that getting all of the bloodwork required adds up very fast, so I’m looking to do it through insurance first.

Jumping in with my latest oral body temperatures. Not much else to report as of late; still taking 75mcg of synthroid along with 25mcg of tiromel. I actually have been seeing a slight improvement in libido and ED, although not nearly enough to the point that I would be confident to have sex without using some type of PDE-5 drug.

How do you feel?

Interesting that most of your pm temps don’t make it past 98.

You have recent thyroid labs while you are on thyroid meds?

Also state your full protocol. I Don’t know if you are on trt. Did not read up

That’s the thing, besides the lack of libido and ED (which started me on this journey), I feel fine. No real fatigue, brain fog, etc. My hands and feet do consistently feel cold though, but I chalk this up more to the Canadian winter than I do hypothyroidism…

No, I am not on TRT, as my endo diagnosed me with Hashimoto’s Thyroiditis as the cause for these issues and started me on thyroid medication. My endo was very traditional and very clueless, as obviously I’m still having these issues after being on thyroid medication for over a year. I was also diagnosed with sleep apnea a few years earlier and use a CPAP machine every night. My current protocol is 75mcg Synthroid first thing upon waking, and I also self-medicate with 25mcg of Tiromel as my endo is an idiot and doesn’t believe in the connection between T4 and T3. I literally had to add rT3 testing to my lab requisition and pay for it out of pocket just to get it checked the one time.

Here are my latest blood work results. I’m a little concerned about my eosinophils increasing on both recent tests, however, I have read that taking Synthroid can cause this. Interesting to see that my TSH is actually low now after being on 0.75mcg of Synthroid and 25mcg of Tiromel. I self-medicate with T3 because my endo is uneducated in regards to fT3 and poor conversion, but I may have to stop taking this, as low levels are also detrimental. Creatine kinase is also a little high, but my creatinine is down this time, which is odd because I am currently taking 5g creatine/day. I will definitely take a break from it once my current tub is gone. Still no improvement in the ED or libido department, but thankfully, I am being referred to a urologist and will try my luck with going this route. Wish me luck!

Your estrogen is too low, no wonder you’re having erectile troubles.

Are you still off of the AI?

@systemlord I find this very interesting because as of this blood test I hadn’t been on any exogenous testosterone, an AI, or anything for that matter for over four months. I do take some natural supplements such as zinc, vitamin E, and occassionally maca, and also eat a fair amount of broccoli (DIM). I also seemed to have higher estrogen most of my life, as I remember my nipples being quite sensitive throughout puberty.

How would I naturally bring my estrogen levels up? And would this help with my ED?

@NH_Watts yes sir. I have been off everything for just over 4 months now, so I find it quite odd that my estrogen is low? I was always under the belief that my estrogen was naturally higher than normal, as I remember getting sensitive nipples throughout puberty. Any ideas as to how I would naturally increase it?

Checking in with my latest oral body temperatures, which have seemed to improve slightly since I adjusted my dose of T3. After getting TSH reading that was too low at the end of January, I decided to reduce my self-medicated T3 dosage to 6.25mcg/day to 25mcg/day. I will have bloodwork done again at the end of April to see if this brings my TSH levels up into a healthy, normal range. I am also quite surprised to see that @systemlord commented that I am experiencing erectile issues because my estrogen is too low. I haven’t taken any exogenous testosterone, an AI, or anything else for that matter for over 4 months now, just natural supplements that would lower estrogen such as zinc, vitamin E, and occassionally maca. I also eat a lot of broccoli (DIM), but quite frankly I’m shocked that my estrogen would be too low as I seemed to be more sensitive to it growing up as indicated my by nipples being sensitive all throughout puberty. Any ideas as to how I would naturally increase my estrogen levels?

TRT is the only way, testosterone converts to estrogen. Your not supposed to dial in thyroid on TSH alone, but fT3 and rT3. TSH is a poor indicator for thyroid status, fT3 is more closely related to symptoms than TSH.

to see if this brings my TSH levels up into a healthy, normal range.

fT3 shows what’s going on at the cellular level. I have never heard anyone try to bring “TSH up to healthy levels”, were are always trying to decrease it and that statement suggests you don’t know what you’re doing. TSH is best lower around 1.0 naturally, lower when using thyroid medicine.

My TSH is under 1.0 naturally as I have a fast metabolism.

You can take your own labs without a doctor. There are sites out there. I use discounted labs. Find a hormone specialist stop wasting your time on these endos and PCPs.

Thankfully my endo has me doing blood work at the end of April to test fT3 and rT3, so I should have a clearer picture of things then.

And obviously I’m not a doctor, but I wouldn’t say that I don’t know what I’m doing. While it is expected to have a TSH lower than 1 on thyroid hormone, having your TSH too low puts you at an increased risk for heart disease, abnormal heartbeat patterns and bone fractures. A doctor that I am working with in the US agreed with this and actually he me cut my current T3 dosage in half, even though I wasn’t experiencing any hypo symptoms. I’m really just trying to find my sweet spot at this point…

Checking in with my latest oral body temperatures. I’m starting to notice a slight improvement in ED symptoms as of late. I take PDE-5 inhibitors quite regularly, but am still noticing the effects long after they have worn off, which tells me that perhaps I am dialing in my thyroid hormone dosage. Currently taking 75mcg of levo/day with 6.25 self-prescribed Tiromel. I also have an appointment finally with a urologist on May 21st to investigate any physical issues that could be the cause of my problems.
